To Make a Referral
If you would like to make a referral to the Regional
Consultation Program for a child in Early Intervention or a child care
program, there is a referral form for Early Intervention Programs and
a referral form for Child Care Providers and Resource and Referral Agencies
to use. Please complete the appropriate forms and send to Step One Regional
Consultation Program, 500 Victory Road, Quincy, MA 02171
Directions for Completing Referral Form
for Early Intervention Service Coordinators:
1. Please discuss the referral with the family,
fill out questions on the form and have parent sign the release portion
of the form.
2. The service coordinator will add the referral
to the RCP to the IFSP.
3. The service coordinator will send the completed
referral form and the developmental assessment to the RCP by fax or
mail.
4. Once the information is received, the RCP coordinator
will contact the EI Service Coordinator within 3-7 days to schedule
the consultation with the family, and discuss the specific needs of
the child and family.
5. The RCP consultant will meet with the child
and parents and service coordinator at the childs home or in a
community setting.
6. After the first consultation, the service coordinator,
RCP consultant and parent can determine whether more services and/or
resources will be needed from the RCP.
Directions for Completing a Referral Form for
Child Care
Providers or Child Care Resource and Referral Agencies:
1. Discuss the consultation referral with the
parent. The parent must agree with the provider regarding the need for
a consultation.
2. The child must be enrolled in an Early Intervention
Program in order to receive RCP services. The RCP staff can assist the
childcare staff or parent in enrolling in an Early Intervention Program.
3. Complete all questions on the referral form
